Centreville HS Official Transcript Request form for students who've graduated/left within last 5 years

This form is intended for students who have graduated from or left CVHS within the past five years and need their transcript for employment purposes, university transfer applications, new college applications, military, etc. If you need an unofficial transcript for personal reasons, please email a request to [email protected].

If you graduated more than five years ago, please contact the Central Office Records Office for your transcripts at 703-329-7741 or [email protected].

Directions:

Submit the IS-111 form https://www.fcps.edu/sites/default/files/media/forms/is111.pdf, allowing CVHS to release your school record. The form only needs to be completed and submitted one time. The form must be completed, signed, and emailed to [email protected]. Complete the email subject line as follows: "IS-111: Year you left CVHS, Last name, First name".
Complete the following form in its entirety. Submit a new form for each separate transcript request. Allow for 3 weeks processing time. Please note CVHS will not mail Official transcripts to students’ homes or email addresses.

Please email my Official transcript directly to the Institution's official email address below. I understand an official transcript cannot be emailed to me.

Send via Common App

I am submitting my college (transfer) application(s) via Common App and request you send my transcript via Common App. I added my counselor to Common App already.

Send directly to university/institution via Parchment or school portal.

I applied to the University directly through their website. Please send my transcript to them electronically.

Send transcript via USPS mail to university/institution.

Please mail my Official transcript to the institution's mailing address below. CVHS will not mail transcripts to personal addresses.

Pick Up

I will pick up an Official, sealed copy of my transcript at CVHS Student Services. I understand I will be emailed when it is available for pickup.
