Below you will find the forms you will need to start the new school year.
Please complete all of the required forms by September 6, 2024.
REQUIRED FORMS FOR ALL STUDENTS
- All students must have accurate emergency care information. FCPS is offering a new online option where parents can easily update their child’s information! The Online Verification Update packet or OVU will allow parents to review and update phone numbers, emergency contacts and health information (such as health conditions and immunizations) for students in grades PS/HS and kindergarten through 12th grade. Enrolling parents* will be able to access OVU through their SIS ParentVUE account. In order to log into Parent View it is necessary to have an active SIS Parent Account. If you need a SIS Parent Account, please contact Cindy Coreas Chavez at c[email protected]. If you are new to FCPS you will receive an activation letter in the mail.
*The enrolling parent is the natural or adoptive parent or legal guardian with whom the student lives the preponderance of the school week and who enrolled the student in school. Regulation 2240.

Digital Consent Permission Form
- Please take a moment at the beginning of the school year to review and provide your consent through our easy-to-use Parent Digital Consent system.
Optional Forms
As part of our ongoing effort to help schools with transitioning from paper to electronic forms, we are excited to announce that the following opt-outs will be available in SIS ParentVUE:
- Tutor.com Access (was available April 2022)
- SEL Screener Participation
- Counseling Opt-Out
- Transportation Intent
- Accept FCPS Device
- Home Internet Access
Any parent/guardian that is associated with a student, and has a SIS ParentVUE account, can update these opt-outs as needed by going to the “Student Info” tab and clicking “Edit Information.”
Parents that do not have SIS ParentVUE accounts can continue to use paper forms or other notification methods.
FCPS Privacy and Opt-Out Forms

Family Life Education Opt-Out Forms
- Information about Family Life Education can be found on the FCPS website for Grades 9-12. Complete this form only if you wish for your child to be opted out. Please email by September 6 to the staff member listed below:

Military-Connected Student Form
- Only to be filled out if you are a military family. This form should be completed by September 6, 2024. Please email form to Emiley Slebrch at [email protected]
Free and Reduced Lunch Application
- If applicable, apply for Free and Reduced Lunch or view information about the FCPS Food and Nutrition Services.
Homeless Student Referral Form
- If applicable, please complete this form and email to Carolina Diaz at [email protected].

- FCPSOn Opt Out Form - If applicable, students who wish to provide their own laptop in place of an FCPS issued laptop will complete and return a signed Opt-Out Form to Paul DesVergnes at [email protected]
- Student Device Guidelines
- Parents will be invoiced for the $50 technology support fee in October 2024 through MySchoolBucks.com.
- Parents can use this online payment system to check balances and add money to school cafeteria accounts, to make payments to the school for various school related items and to receive low account balance notifications.
